If it's real, it's yet another sign that as long as there are people, there will be bureaucrats issuing paperwork.

They used to quarantine them when they came back from the moon also.
Must've worked, no one got sick from any moon bugs. I can understand why they'd want to err on the side of caution, though.

The Smithsonian has the Apollo 11 Quarantine Airstream in its collection. And as far as I can determine, the form's legit.

Hey Doug,

The last time I went onboard the USS Nimitz, which is on display in your backyard in Alameda , they had one of the quarantine airstreams and an Apollo capsule.

Nimitz, of course, was the ship that scooped up the capsule when the crew returned from the moon.

Danny, it was the USS Hornet that recovered the Apollo 11 capsule. Nimitz wasn't in service until around `75.

Yeah, the carrier parked at Alameda is indeed the Hornet, and I do hope to visit it's museum some time.
The Nimitz is still in service (until around 2025) and its current home port is Everett, WA.
The Hornet museum is only a couple of miles from the Nimitz Freeway, however.
